Watch the drop happen
Open the Gamepad tester: check your controller online and leave it visible on a second screen or window while you play. The header shows the polling rate, the number of input reports per second. A healthy wired pad holds 125 to 250 Hz steadily; a Bluetooth pad 60 to 125 Hz. If the number sags toward zero in the seconds before a disconnect, the link is degrading gradually, which means interference or a weak battery. If it stays solid and then the controller vanishes outright, the cause is a power cut: a loose cable, a USB port going to sleep or the battery contact.
Battery and power first
Xbox controllers on AA batteries disconnect when voltage sags under rumble load, even if the battery meter still shows one bar. Fit fresh cells or a charged play-and-charge pack. DualSense and DualShock batteries are internal; a pad that only disconnects when vibrating has a tired battery that cannot supply the rumble motors' peak current. On PC, Windows can power down USB ports to save energy: in Device Manager, open each USB Root Hub, Power Management tab, and untick Allow the computer to turn off this device.

Bluetooth interference
Bluetooth shares the 2.4 GHz band with Wi-Fi, wireless mice, USB 3 ports and microwave ovens. A USB 3 hard drive or a Wi-Fi dongle plugged in right next to the Bluetooth adapter is enough to cause drops. Move the dongle to a front port or a short extension cable, move the router or switch it to 5 GHz, and keep the controller within three metres with no body between it and the receiver. If drops only happen on one computer, its Bluetooth adapter is the weak link; a cheap Bluetooth 5.x USB dongle solves most cases. For Xbox pads, the official Xbox Wireless Adapter uses its own protocol and is far more stable than Bluetooth.
Firmware
Xbox Series and One controllers have had several firmware updates specifically for disconnects. Update through the Xbox Accessories app on Windows or on the console. DualSense firmware updates ship through the PS5 system software or the Firmware Updater for DualSense on PC. Switch Pro Controller firmware is updated from System Settings → Controllers and Sensors.
Worn USB port
If a wired controller disconnects when the cable is nudged, the port on the controller has worn or cracked solder joints. Confirm by holding the cable still: if drops stop, the port is at fault. Micro-USB ports on the DualShock 4 and USB-C ports on the DualSense are both replaceable with hot-air soldering equipment; the charging guide explains the symptoms in detail.

Frequently asked questions
My controller disconnects only in one game.
The game is probably triggering heavy rumble or the controller is being put to sleep by an idle timeout the game does not reset. Try disabling vibration in that game and check the controller's sleep setting.
Wired Xbox controller keeps disconnecting on PC.
Change the cable first, then disable USB selective suspend in Windows power options. If it persists, try another USB port on the back of the PC.
Does a disconnecting controller need repair?
Only if it drops out on every device, with a new cable and full battery, or when the port is physically loose. Otherwise it is interference or power management.
